Default 94 Civic: boot the clutch slave cylinder goes into??

There's an additional rubber boot that covers the boot end of the slave
cylinder and attaches into the tranny housing. Since that isn't
particularly supple any longer I'd like to replace it while I'm
replacing the slave cylinder. I'm pretty sure it's the "release fork
boot" marked item #7 in the MT Clutch Release section at slhondaparts:

Also, does it simply pull out of and push back into the tranny housing,
held in by a raised lip (kinda looks that way), or is there some sort of
attachment I'll have to release and replace? I don't want to start
pulling on it before I know better, given that it's tucked behind the
radiator and I can't get a good look at it easily.

One mo does a new bleeder screw and cap come along with the slave
cylinder, or must it be purchased separately? From the fact that the box
marked item #9 in the diagrams encloses these items, I'd like to think
they're included. Can anyone confirm? Thanks.
